As part of our commitment to providing quality care to your family and others, we do ask that everyone in the office follow certain conduct guidelines.

This notice describes how medical information about you may be used and disclosed and how you can get access to this information.

Does your child need a specialist? Please reach out to us before scheduling an appointment. We collaborate with trusted specialists who keep us informed about our patients' needs.

At Pediatric Associates at Northwoods, we are committed to providing the best possible medical care for your child, from infancy through their young adult years. 

The MIIS is a statewide system that keeps track of the immunization records for families.

Please notify us at least two business days before your prescription runs out.

We will be happy to provide children with a school/work note if they were seen at our office.
