A Medical Home empowers families by providing the following:

  • Accessibility: We're readily available and easy to reach, utilizing technology for seamless communication. Our office is open most days, and a clinician is always available by phone.
  • Family-Centered Care: You are the expert on your own child. We make all decisions  in partnership, tailored to your child's unique needs. 
  • Continuity: Your child will have a designated PCP who will oversee their care from infancy to young adulthood. While our staff works as a team, we always loop care back to your PCP.
  • Comprehensive Care: We provide both preventive and primary care, acting as a central resource for your child's other healthcare needs.
  • Coordination: We collaborate with you and other specialists to create and implement a comprehensive care plan, especially if your child has complex health care needs.
  • Compassion: We prioritize your child's emotional well-being along with their physical health. 
  • Cultural Sensitivity: We celebrate the uniqueness of families. We welcome all families to our practice, irrespective of race, ethnicity, language spoken at home, religion, special health care needs, socioeconomic status, sexual orientation, gender identity, and other attributes.

  • Communicate your child’s needs and any changes in health.
  • Keep us informed about care received outside our practice. 
  • Keep your child’s medical appointments. 
  • Follow the agreed-upon care plan and let us know of any challenges.
  • Understand your insurance coverage.
